New Members Forms and Processing December 2005

All of the forms(except fingerprint cards) used to process a new member can be obtained herejust click on the desired form.

The forms will appear in .pdf format which requires Adobe Acrobat Reader©. If you do not have Adobe Acrobat reader© it can be obtained by clicking on the logo at the District page. Most files include the instructions for completing that form. The selected form should be saved to a file on your computer. It can then be recalled for printing, copies can be made, it can be forwarded to a prospective new member as an e-mail attachment, etc.

Some flotillas have collected all of these forms on a CD and provided that CD to the prospective member.

The new member package consists of two parts. The enrollment section consisting of the enrollment form, interview form, and several other items and the Personal Suitability Investigation (PSI) most of which are listed on the current enrollment application section V.

The following are the elements of the enrollment section.

4. The New Member ID Card form http://www.uscgaux-d8cr.org/pdf/IDCARDFORM.pdf including a head photo with a red background in jpeg format. The photo should be on a floppy disk.

5. The Direct Deposit Form. http://www.uscgaux.org/~forms/archive/fms2231-cgaux-f.pdf Reimbursement for any activity is only available via electronic deposit.


The following are required for the Personnel Suitability Investigation (PSI) portion of the New Member Package

1. USCG Auxiliary PSI Package Checklist. http://www.uscgaux-d8cr.org/security/SECCEN-CHECKLIST.pdf Check the boxes in the appropriate section. If any box in section 3 is checked DO NOT submit the package. Contact Jack Granger or Parlee Harper at the DIRAUX office.

2. The Verification of U.S. Citizenship form http://uscgaux-d8cr.org/security/USCG-AUXILIARYctizenship.pdf with copies of the appropriate documents. This form must be signed and dated by the Auxiliary Officer (typically the FSO-PS, FC or VFC) who personally examined the original birth Certificate, passport or other suitable document.

3. Two (2) FD-258 USCG fingerprint cards with original signatures and completely filled out. NOTE: This is the only form NOT available on-line. Contact DIRAUX.

4. IF the prospective (or current) member has a prior or current security clearance from any US organization complete a Prior/Current Clearance Cover Sheet

5. one (1) SF form85 (page 6 only) http://www.cgaux.info/g_ocx/administration/security/85release.pdf Authorization for release of information with original signature.
(1) American (by birth) citizens need to fill out 1 through 6 and 13.a & 13.b
(2) Naturalized citizens need to fill out 1 through 6 and 13.a., 13.b & 3.c
(3) Dual citizens need to fill out 1 through 6 and 13.a., 13.b., 13.c & 13.d
(4) Coast Guard Security Center (SECCEN) will fill out remaining blocks.

6. one (!) OFI Form 86C http://www.cgaux.info/g_ocx/administration/security/SACAgreementBVS-INSw-citizenship2.pdf Special agreement check including mothers full and maiden name and father’s full name.

To compete the application process check to make sure all the required documents are complete and have original signatures and all required attachments are included. Mail the package to the following address until instructed otherwise.

Don’t forget to collect a dues check and have it deposited in the flotilla account.

Some units request that you send a copy of the enrollment form – just the one page – to the SO-FN. When the PSI process is completed the flotilla commander will be notified and the DSO-FN and SO-FN will send appropriate bills to your flotilla.
