USCG Offered Aircraft Facility Inspection Form

If you currently have an AUX aviation facility or are offering a new facility for use, please use the following form to notify us of you need for a facility inspection.

We will then contact you after submission of this form to set a date for the actual inspection using the data you have provided here.

We will provided a pre-filled ANSC 7005 (1-05) based on the form data.

Please have the following at the time of physical inspection:

Airworthiness Certificate
Registration Documents
Aircraft logbooks with all inspections and checks up-to-date
Weight and Balance
VHF radio if it is not permanently installed
Operating Limitations
First Aid Kit
Required safety equipment per ANSC 7005 (1-05)
